Contact the Fire Department and San Joaquin Valley Air Pollution Control District <br /> for applicable permits. <br /> 2. The "Certificate of Compliance for Underground Storage Tank Installation Form <br /> C' must be completed and submitted to this office upon completion of work in <br /> order to certify that the repair was done properly. <br /> 3. At least one person who holds the International Code Council UST <br /> Installer/Retrofitter certification must be onsite when work is performed. <br /> 4. Any person installing or testing any component must hold a certificate of training from <br /> the manufacturer of that component. <br /> 5. Contact EHD to schedule the following appointment(s): <br /> a. Inspection of sump to verify repair <br /> 6. If concrete is broken above the tank, a tank integrity test must be performed by a <br /> certified tank tester once concrete is poured, prior to the contractor signing off of <br /> the jobsite. This office must receive results within 7 days of completing the work. <br /> 7. Allow 48-hour advance notice for scheduling inspections with this office. <br /> 8.
