The record may <br /> take the form of a log, invoice, manifest, bill of lading, or other ship ng document, and shall include the <br /> following: <br /> - Name & address of destination facility <br /> - Quantity &type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> Complete and submit a copy of the Return to Compliance Certifica n to the EHD by June 23, 2011 <br /> along with a statement documenting the corrective actions that ha been or will be taken for each <br /> violation and any supporting paperwork. <br /> **Facility generated less than 5 tons of hazardous waste in 2010. <br /> Note: (corrected on site) An emergency coordinator and modified ntingency plan information is <br /> lacking. There must be at least one emergency coordinator on sit r on call to coordinate emergency <br /> response measures, and the following information must be posted y a phone: the name and phone <br /> number of the emergency coordinator; location of fire extinguisher spill control equipment, and if <br /> present, fire alarm; and the phone number of the fire department, less the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the requi d information by a phone.
