Well Destruction Report <br /> Shell Service Station <br /> 420 W Kettleman Lane <br /> Lodi, California <br /> WIC #204-4416-0202 <br /> Dear Mr Knoll <br /> This report documents the destruction of four ground water monitoring wells at the above <br /> referenced site (Plates 1 and 2) The work was performed to comply with San Joaquin <br /> County Public Health Services (SJCPHS) case closure requirements outlined in the <br /> SJCPHS letter dated May 13, 1996 <br /> Site Description <br /> The site is an active retail gasoline service station located on the southeast corner of <br /> Kettleman Lane and Hutchins Road in a commercial area of Lodi (Plates 1 and 2) <br /> The existing facilities include a station building with car wash facility, five <br /> dispenser islands, and three underground storage tanks <br /> Monitoring Well Destruction <br /> On October 15, 1996, Wells MW-1 through MW-4 were destroyed by pressure grouting <br /> Each well was filled with cement grout to the top of the casing A pressure tight hose <br /> fitting was fitted to the top of the well casing and additional cement grout was pumped into <br /> the well under pressure After pressure grouting was completed, the wellhead area was <br /> backfilled with concrete to grade A representative from SJCPHS was present on-site to <br /> inspect the pressure grouting operations <br /> Fnvlros, Inc 270 Perkins Street Sonoma California Tel 707/935 4850 <br /> P 0 Box 259 95476 0259 Fax 707/935 6649 <br />
