Box 3 88 <br /> Stockton, CA 95201-0388 <br /> Subject: Well Destruction Permit Application, Ekholm Property, 1702 Jackson Street, <br /> Escalon, California <br /> Dear Ms. Turkatte: <br /> As you are aware, case closure has recently been granted by the San Joaquin County Public <br /> Health Services, Environmental Health Division (PHS/EHD) for Ekholm property located at 1702 <br /> Jackson Street, Escalon California (Figure 1). Smith Environmental Technologies Corporation <br /> has been requested by Mrs. Margaret Ekholm to conduct final closure activities at the site <br /> including abandoning nine existing groundwater and vapor wells located at the site. Smith <br /> Environment proposes to abandon the wells using the following described methodologies. The <br /> locations of the wells are shown on Figure 2. <br /> The casing and sandpack for groundwater monitoring wells MWA through MW-4 will be <br /> overdrilled by a licensed drilling company. Subsequent to overdrilling, the borings will be filled <br /> with a neat cement grout from the bottom of the borings to 1 foot below grade using tremie hose. <br /> The top 1 foot of the borings will be backfilled, flush with the ground surface, with concrete, <br /> asphalt patch or native soil to match the exiting surface. <br /> Vapor extraction wells VW-1 through VW-5 will be abandoned by pressure grouting the casing <br /> and sandpack with a cement/bentonite grout mixture, leaving the casing in-place. The well boxes <br /> will be removed, if possible, and the top two to four feet of each boring will be backfilled, flush <br /> with the ground surface, with concrete, asphalt patch, or native soil to match the existing surface. <br /> Attached please find a completed well destruction application with check no. 1020 for the permit <br /> fee ($60)to perform this work. <br /> 1260 Graphics Drive Modesto,CA 95351-1502 (209)579-2221 Fax(209)579-2225 <br /> Engineering - Consulting Remediation Construction <br />
