++[ J..
1. 1
S H
Field activities will be summarized in a quarterly monitoring report and submitted to you in
draft form. After receiving and incorporating any comments, Smith Environmental will
submit the final report to the PHS/EHD and RWQCB.
Task 3: Verification Sampling and Closure Report
E'
f When periodic monitoring of the VES indicates that soil remediation has been completed,
Smith Environmental will initiate site closure activities. A workplan outlining proposed field
f methods will need to be submitted to PHS/EHD for review prior to initiation of drilling
activities. Subsequent to PHS/EHD approval, a geologist will supervise the drilling of three
confirmation borings to depths of approximately 70 feet below grade. Drilling will be
conducted by a State licensed driller using a drilling rig equipped with hollow stem augers.
Soil cuttings will be screened in the held for the presence of residual petroleum hydrocarbons
using a photoionization detector (PID), or equivalent. A minimum of three soil samples will
be collected from each boring to confirm successful remediation of subsurface soils. Soil
samples will be collected into pre-washed brass tubes, placed on ice and transported under
chain-of-custody to a state certified analytical Iaboratory. Soil samples will be analyzed for
the presence of BTEX using EPA method 8020, TPHg using EPA method 8015 modified, and
soluble lead using the Waste Extraction Test (WET). Drill cutting will be stored on site on
plastic pending laboratory results. Disposal costs are not included in this estimate.
If analytical results confirm successful remediation of subsurface soils, Smith Environmental
will prepare a closure report summarizing site activities and presenting all data collected
during the project and initiate well abandonment and VES decommissioning activities.
Task 4: Well Abandonment and VES Decommissioning
Upon receipt of site closure certification, Smith Environmental will prepare a brief letter
workplan and permit application for submittal to PHS/EHD. After receiving approval, a
geologist will oversee the abandonment of all vapor and groundwater wells by methods
approved by California Well Standards. The well casing and sand pack will be overdrilled by
a licensed drilling company. The borings will then be grouted from the bottom of the boring
to the ground surface using neat cement.
E Smith Environmental will decommission the vapor extraction system including removing vapor
extraction system piping from the site, unhooking the blower, taking down the emission stack,
shutting off the electrical power, and removing all salvageable gauges, valves and parts and
returning them to the owner. This task does not include major restoration of the building on
site.
I. A brief letter report summarizing well abandonment activities and containing well destruction
completion notices will be prepared and submitted to you in draft form.
