CA 94244-2120 SAMPLE <br /> 916) 739-4352 <br /> O <br /> J <br /> Responsible Parties <br /> COST RECOVERY FOR OVERSIGHT OF LEAKING UNDERGROUND STORAGE TANK (UST) CLEANUP <br /> When the local agency included your site in the pilot program for leaking <br /> underground storage tank (UST) cleanup, you received a letter explaining that the <br /> State Water Resources Control Board (State Board) would bill you for public costs <br /> of cleanup oversight. Enclosed is a- bill for local and Regional Water Quality <br /> Control Board (Regional Board) supervision of cleanup at the site listed in the <br /> enclosed bill. You will be billed at a future date for State Board program <br /> management. <br /> This bill contains site-specific and program management costs. Site-specific <br /> charges are based on technical work related exclusively for the site and are <br /> listed on the enclosed °Itemized New Charges" sheet. The column entitled :A <br /> (for activity) contains a three-digit number. A description of invoice activity <br /> codes is printed on the reverse side of this letter. Site-specific charges also <br /> cover laboratory analyses of samples, if any were taken by the local agency <br /> staff. Program management costs include administrative work and laboratory <br /> consultation related to the general cleanup program. There are local and state <br /> program management charges. Local program management includes both start-up and <br /> ongoing activities. State program management includes both State Board and <br /> Regional Water Quality Control Board (Regional Board) work. <br /> Your share of program management and laboratory consultation charges depends on <br /> how much site-specific time was charged to your site, compared with all site- <br /> specific time charged by the local agency. For example, if 10 hours were charged <br /> to your site, and 1,000 hours were charged to all sites, then your share of <br /> program management costs would be 10 divided by 1,000 or one percent. <br /> Your payment is due within 30 days of receipt of this bill. Please return one <br /> copy of the invoice with your payment.
