Box 5811 San Mateo CA 94403 RE: ARCO STATION #434 541 KETTLEMAN; L DI 95240 The San Joaquin County Public Health Services, Environmental Health Division has recently been included in the State Water Resources Control Board (SWRCB) UST Pilot Program for oversight of assessment and clean up activities at contaminated underground tank sites. A proposal for initial assessment and/or remediation for the above referenced site has been submitted to this office. All proposals and reports submitted musi be accompanied by a Letter of Acknowledgement from the responsible pa ty ( ies) which states, at a minimum, the following: I (We) declare, under penalty of pe jury, that the informatiofi and/or recommendations contained iT the attached proposal or report is/are true and correct, anc that all work and reports which required geologic or enginee ing evaluations and/or judgments have been performed undev the direction of an appropriately registered or certified professional. The attached proposal or report utilizes the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground Tanks" and "Appendix A` reporting format. The LOFT Manual has also been utilized as a guidance document.
