The following is an itemized list of hazardous materials violations that have not been <br /> addressed for FRESH INNOVATIONS CALIFORNIA as of February 11 , 2020 . <br /> Open violations from November 26 , 2019 inspection <br /> Violation #7 - Failed to establish and /or submit emergency response procedures for a release or threatened <br /> release . <br /> The emergency response procedure has incomplete information . The Emergency Contingency Plan in CERS listed <br /> the CUPA phone number(20 468-342 ) . The correct phone number for the CUPA is 209468-3420 . The San Joaquin <br /> County General Hospital phone number listed as 20468600- The correct phone number for SJC General Hospital is <br /> (209 )468-6000 . A business plan shall include the following emergency response procedures for a release or <br /> threatened release of hazardous materials , including , but not limited to , the following : <br /> - immediate notification of local emergency personnel and the Environmental Health Department ( EHD ) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property , or the environment <br /> - evacuation plans and procedures , including immediate notice , for the site <br /> Immediately log into the California Environmental Reporting System ( CERS ) at http : //cers . calepa . ca . gov/, upload the <br /> correct or updated information , and submit to the EHD for approval . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date ) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel . <br /> There were no records of all the employees training at the time of the inspection . The only training found during the <br /> inspection is the ammonia evacuation training . The business plan shall include provisions for ensuring that <br /> appropriate personnel receive initial and annual refresher training . All employees shall be trained in safety <br /> procedures in the event of a release or threatened release of a hazardous material , including , but not limited to , <br /> familiarity with the following plans and procedures : <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> ( B ) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons , property , or the environment . <br /> ( C ) Evacuation plans and procedures , including immediate notice , for the business site . <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years . <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Page 1 of 1 <br />
