The following is an itemized list of hazardous materials violations that have not been <br /> addressed for FRESH INNOVATIONS CALIFORNIA as of February 11, 2020. <br /> Open violations from November 26, 2019 inspection <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedure has incomplete information.The Emergency Contingency Plan in CERS listed <br /> the CUPA phone number(20 468-342).The correct phone number for the CUPA is 209-468-3420.The San Joaquin <br /> County General Hospital phone number listed as 20468600-The correct phone number for SJC General Hospital is <br /> (209)468-6000. A business plan shall include the following emergency response procedures for a release or <br /> threatened release of hazardous materials, including, but not limited to, the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> There were no records of all the employees training at the time of the inspection.The only training found during the <br /> inspection is the ammonia evacuation training.
