(AA&A), on your behalf. The addendum was submitted in <br /> response to a request from the EHD during a telephone conference attended by AA&A and <br /> EHD personnel on 29 March 2011. <br /> In the addendum, AA&A proposes to add 5 additional dual-phase extraction (DPE) wells to the <br /> high-vacuum dual-phase extraction (HVDPE) system proposed by AA&A to treat <br /> contaminated soil and groundwater at the above-referenced site. The proposed 11 DPE well <br /> locations are illustrated on Figure 1 of the addendum. In an email dated 24 May 2011, AA&A <br /> revised the screen interval of the DPE wells from 4 to 20 feet below ground surface (bgs), to 6 <br /> to 20 feet bgs to adjust for corrected depths to wader In monitoring well MW-3R. In the original <br /> Corrective Action Plan, dated 18 October 2010, AA&A had proposed collecting soil samples at <br /> 5-foot intervals between 15 and 25 feet bgs for lithologic evaluation and analysis for total <br /> petroleum hydrocarbons as gasoline (TPH-g); total petroleum hydrocarbons as diesel <br /> (TPH-d); benzene, toluene, ethylbenzene, and total xylenes (BTEX); methyl tertiary-butyl <br /> ether (MTBE), ethyl tertiary-butyl ether (ETBE), tertiary amyl methyl ether (TAME), di- <br /> isopropyl ether (DIPE), and tertiary butyl alcohol (TBA) during the advancement of the soil <br /> borings. In the opinion of the EHD, soil samples should be collected at 5-foot intervals <br /> between 5 and 20 feet bgs to verify the presence or absence of contamination. With these <br /> revisions, the EHD approves the two 30-day HVDPE events proposed by AA&A in the <br /> Corrective Action Plan using the newly installed extraction wells. <br /> On 12 July 2011, the EHD posted public notification of pending approval by this department of <br /> the Corrective Action Plan to implement remediation at this site.
