It is not necessary to provide the social security number, the <br /> driver's license number, nor the home address; but it is important that you <br /> include the Tax ID number. <br /> The business/facility information is once again the release site (Frontier <br /> Transportation). <br /> I'm not sure what you mean by"Mailing address for health permit". You must <br /> submit two permit applications since two different parcels are involved in the <br /> fieldwork. Both permit applications will be sent to my attention at the San <br /> Joaquin County Environmental Health Department. One check can <br /> be submitted to include both permit application fees. <br /> The Account Address is the release site responsible party. <br /> It is most important that you understand that the applicant name applies to <br /> whomever is going to be responsible for paying any bills that exceed the permit <br /> fees. You should not sign this unless you are willing to be responsible for bills <br /> that exceed the initial permit fees. I would recommend that you have the <br /> responsible party sign the application. The driver's license number is not <br /> necessary. If someone from Ami Adini &Associates signs the application, then <br /> the person who signs the application is responsible for any additional charges <br /> associated with the proposed fieldwork. <br /> I hope I have answered all of your questions.
