You must submit two permit applications <br /> since two different parcels are involved in the fieldwork. Both permit applications will be sent to my attention at <br /> the San Joaquin County Environmental Health Department. One check can be submitted to include both permit <br /> application fees. <br /> The Account Address is the release site responsible party. <br /> It is most important that you understand that the applicant name applies to whomever is going to be responsible <br /> for paying any bills that exceed the permit fees. You should not sign this unless you are willing to be responsible <br /> for bills that exceed the initial permit fees. I would recommend that you have the responsible party sign the <br /> application. The driver's license number is not necessary. If someone from Ami Adini &Associates signs the <br /> application, then the person who signs the application is responsible for any additional charges associated with <br /> the proposed fieldwork. <br /> I hope I have answered all of your questions.
