I just checked and the work plan has not <br /> arrived at our office yet. I was hoping to receive the hard copy so that I would not have to <br /> print the work plan. Please confirm that the work plan was mailed to the address below. <br /> I have appended a copy of the well destruction permit application, the Licensed Contractor's <br /> Declaration (LCD) form and the Master File Record (MFR)form. All three forms must be <br /> completed once the well destruction work plan has been approved. The Environmental <br /> Health Department's website is listed at the top of the permit application so that you can get <br /> additional information. There is a$375 fee per parcel for setting up the account, issuing the <br /> permit and doing the field inspection. Also the fee for destroying each well is $125.00. 1 <br /> would prefer that you not submit the permit application, LCD and MFR until after the work <br /> plan has been approved. <br /> Also, have you uploaded the work plan to GeoTracker? <br /> Please let me know if you have any questions concerning how to fill out the forms. In our <br /> meeting,we talked about the proposed scope of work that we had discussed in our <br /> June 26th meeting and I had finalized in my work plan dated July 1st.The RPs signed a <br /> contract with us(EnviroMonitoring Services)to proceed with two action items that <br /> we had proposed: 1) water pumping test, 2)abandonment of the un-necessary wells. <br /> The third action item which is groundwater remediation will take place after we <br /> complete the water pumping test and design GW remediation system. <br /> 7/12/2013 <br />
