Work Plan <br /> Frontier Transportation <br /> July 23, 2001 <br /> 7. 1) Field Log Book <br /> I <br /> j An accurate chronological recording of all field activities is vital to the documentation of <br /> any environmental investigation. To accomplish this, bound and numbered field logbooks will <br /> be maintained by the field team to provide a daily record of significant events, observations, and <br /> deviations from the work plan and measurements collected during the field activities. The BEII <br /> on Site Supervisors will determine the necessity for any deviations from the work plan. The <br /> ;I records will contain sufficient information so that the work activities can be reconstructed <br /> without relying on the collector's memory . All entries will be signed, dated and made with <br /> waterproof ink. Corrections to the logbook will be made by drawing one line through the error, <br /> initialing and dating. The logbook will always be stored in a secure location. <br /> l 7.2) Chain-of-Custody <br /> The objective of the Chain-of-Custody Program is to allow the tracking of <br /> possession and handling of individual samples from the time of field collection through <br /> laboratory analysis . Once a sample is collected, it becomes part of the Chain-of-Custody <br /> process. A sample is "in custody" when ( 1 ) it is in someone's possession; (2) it is within visual <br /> proximity of that person; (3) it is in that persons possession, but locked up and sealed (e.g. <br /> during transport); and (4) it is in a designated secure sample storage area. <br /> 7.2.1) Chain-of-Custody Record and Request for Analysis Report <br /> Chain-of-Custody records establish the documentation necessary to trace <br /> sample possession from the time of collection to analysis. A serialized Chain-of-Custody and <br /> Request for Analysis Report will be completed and will accompany each batch of samples . The <br /> record will contain the following information. <br /> * Project name and number; <br /> * Request for Analysis control number (for cross reference) ; <br /> * Names of sampling team members; <br /> * Laboratory destination; <br /> * Carrier/waybill number; <br /> * Sample number; <br /> * Sample location and description; <br /> * Date and time collected; <br /> * Sample type; <br /> * Container type; <br /> * Special instructions; <br /> * Possible sample hazards; <br /> 5 <br />
