One copy will accompany the sample <br /> I , J shipment to the laboratory and one copy will be kept with the field logbook. All documents that <br /> accompany shipments will be enclosed in zip-lock bag and taped to the inside top cover of the <br /> I � shipping container. <br /> - Chain-of-Custody and Request for Analysis Reports provide official <br /> communication to the laboratory by listing the particular analysis required for each sample. This <br /> also furnishes further evidence that the Chain-of-Custody is complete. The form will contain the <br /> following information : <br /> l * Cross-reference to the Chain-of-Custody Record; <br /> J * Project name and number; <br /> * Sample number; <br /> + Sample volume; <br /> * Preservative as required; <br /> * Requested testing program; <br /> * Required turnaround time; <br /> * Possible hazard identification; <br /> * Sample disposal requirements; <br /> i _ <br /> The form will be signed and dated by the receiving laboratory sample <br /> management custodian. <br /> _ I <br /> 8.2.2) Sample Identification <br /> Sample labels prevent the misidentification of samples . Following <br /> sample collection, labels will be affixed to each sample container. Labels will record the <br /> following type of information. <br /> * Project name and number; <br /> * Sample identification number; <br /> * Name and sample collector; <br /> * Date and time of collection; <br /> * Analytical parameters; <br /> * Known hazards; <br /> * Pertinent comments; <br /> .
