Shy// DG Grimm f l�s7li f <br /> 425 Larch Road Date by 17 <br /> Tracy, CA 95376 <br /> Mr. Sasson, <br /> As per our telephone regarding the Proposed Site Assessment Testine workplan dated October <br /> 14, 1997, Blakely Environmental Investigations, Inc. has made the following changes: <br /> 1 .) The location of the four monitoring wells will be placed according to the <br /> enclosed figure. One well (MW4) will be placed approximately 180' <br /> downgradient of MW-3. One well (MW-5) will be placed to the southwest of the <br /> shop. One well (MW-6) will be placed approximately 80' upgradient of MW-3 _ <br /> just east of Industrial Way. If evidence of petroleum hydrocarbon contamination <br /> exists from the installation of MW-5, then one well (MW4) will be placed <br /> approximately 30' upgradient of MW-5. If no evidence of petroleum hydrocarbon <br /> contamination exists from the installation of MW-5, then well MW4 will be <br /> placed approximately 80' north of MW-5. <br /> 2. ) The slot size on the perforated PVC casing will be changed from 0. 020-inch slots <br /> to 0. 0104nch slots at the request of Mr. Steven Sasson. Blank PVC will extend <br /> from surface to 6' below grade (bg) and will be perforated from 6' bg to <br /> termination depth. A sand pack of Monterey #3 sand will extend from 5' bg to <br /> termination depth. This will be overlain with a 5' seal of grout comprising of one <br /> 94 pound bag of cement mixed in 540 gallons of potable water. A drilling <br /> permit application and the appropriate fees shall be submitted to the San Joaquin <br /> County Public Health Services, Environmental Health Division (PHS/EHD) <br /> before initiating any on site drilling.
