This will be recorded at <br /> the bottom of the Chain-of-Custody Record and Request for Analysis Report where the persons <br /> involved will sign, date and note the time of transfer. During field operations, each project <br /> - , geologist will act as the custodian for the samples he or she collects. Samples will not be left <br /> unattended unless placed, along with the Chain-of-Custody Record, in a secure container. <br /> i <br /> The Chain-of-Custody Record and Request for Analysis Report is a multi-part <br /> form that allows the record to be kept in duplicate. One copy will accompany the sample <br /> . shipment to the laboratory and one copy will be kept with the field log book. All documents that <br /> accompany shipments will be enclosed in zip-lock bag and taped to the inside top cover of the <br /> -1 shipping container. <br /> - I <br /> Chain-of-Custody and Request for Analysis Reports provide official <br /> communication to the laboratory by listing the particular analysis required for each sample. This <br /> { also furnishes further evidence that the Chain-of-Custody is complete. The form will contain the <br /> following information: <br /> I_ I • Cross-reference to the Chain-of-Custody Record; <br /> • Project name and number; <br /> [ • Sample number; <br /> I_ + • Sample volume; <br /> • Preservative as required; <br /> i- • Requested testing program; <br /> • Required turnaround time; <br /> • Possible hazard identification; <br /> -I • Sample disposal requirements; <br /> The form will be signed and dated by the receiving laboratory sample management <br /> f custodian. <br /> _I 8.4) Sample Identification <br /> J Sample labels prevent the misidentification of samples.
