Prior to field activities; .well destruction <br /> permits were obtained from the San Joaquin County Public Health Services Environmental Health <br /> Division. All work was-;performed in accordance with California Well Standards Bulletin 74-90. <br /> I <br /> On June 10;`1998;°ATC'Associates supervised the.destruction;of.site.monitoring well MW 1 using <br /> pressure grouting methodologies.. All well abandonment activities were performed by V&W <br /> Drilling, C57 license 720904: <br /> Well MW 1 was abandoned by filling the well casing and filter pack with a neat cement grout <br /> utilizing a pressure grouting tool. After the grout level in the.well casing had stabilized, the well <br /> casing and concrete debris-were-excavated out.to one foot below ground surface (bgs), and well <br /> r casing=fragments and -debris--were-removed-fi-om-the-boring... The_boring_was then filled with y <br /> native soil to the ground.surface.- A copy of field notes summarizing-well destruction activities is <br /> contained in Attachment 1. <br /> In correspondence dated May 12, 1998, PHS/EHD agreed with AT&s recommendation to the <br /> surface discharge of approximately 80 gallons of monitoring well purge water. The purge water <br /> was used to water on-site vegetation.
