Failure to do so may <br /> cause premature product failure or create liquid leakage to soil resulting in hazardous spill <br /> conditions. Failure to follow these instructions can result in environmental contamination. <br /> NOTICE: <br /> Save and use tubular fiber concrete form. <br /> GENERAL GUIDELINES <br /> • Protect tubular fiber concrete form and cover protector disc from prolonged direct contact with <br /> water prior to installation. <br /> • Protect cover seal and O-ring groove from entry of foreign material,especially wet concrete. <br /> • DO NOT use a sharp tool in cover O-ring groove.This can damage seal and/or Cover. <br /> • Install cover and cover protector disc,use fiber concrete form prior to pouring concrete. <br /> • Follow carefully the step-by-step installation instructions. <br /> • All threaded joints must be in conformance to applicable codes. <br /> • Periodic scheduled maintenance and/or testing are recommended. <br /> See Preventive Maintenance instructions. <br /> If you have any questions or require assistance prior to or during installation,contact the OPW Customer Service <br /> Department:(513) 870-3219 <br /> }r <br />
