The following is an itemized list of hazardous materials violations that have not been <br /> addressed for OAK PARK ICE ARENA* as of February 14, 2020. <br /> Open violations from June 18, 2018 inspection <br /> Violation #4- Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was not accurate. Matthew Balk no longer works for this facility <br /> and is identified as the Primary Emergency and Environments Contact. This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for 402 A, nitrogen, 408 A, Forane R 502, Calcium Chloride and Propane <br /> were not complete. The common name, quantities, days on site, storage pressure and storage temperature for 402 <br /> A have not been completed; the hazardous material type and DOT information for nitrogen, 408 A, Forane R 502 <br /> and propane have not been completed; the types of containers have not been identified for calcium chloride; The <br /> units of measure for propane is shown as pounds vs. gallons. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The site map does not show north orientation, storm and sewer drains, street access and exit points, emergency <br /> shutoffs, emergency response equipment, and did not include a legend/key to identify the codes used on the map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment.
