NAY 3rM
PERMITIMMES
8300 Utica Avenue, 3rd Floor • Rancho Cucamonga, California 91730 (909) 476-2120 • Fax (909) 476-6819
"Expert Environmental Solutions"
June 1, 2004
San Joaquin County Environmental Health Department
304 E. Weber Avenue, 3rd Floor
Stockton, CA 95202-2708
SUBJECT: BORING PERMIT APPLICATION-375 WEST HAZELTON, STOCKTON, CA.
To whom it may concern:
ALTA EM, INC. has been retained to conduct a routine investigation for potential environmental
issues at the subject site. This investigation is being conducted above and beyond an existing
leaking UST case registered with your agency. The work is exploratory in nature, and is not
based on any known or confirmed environmental issues on the site. The drilling is being
completed to assess for possible petroleum hydrocarbons related to former on-site operations
and UST issues on adjoining properties.
The investigation will consist of drilling 6 or 7 soil borings to approximately 20 feet deep, and
possibly collecting groundwater samples with a hydropunch in selected areas. Soil samples will
be collected at approximate 5 foot depth intervals, and selected soil and/or water samples may
be analyzed for petroleum hydrocarbons, BTEX, MTBE and other fuel oxygenates. Chemical
analytical work would be completed by a State of California Department of Health Services
certified laboratory. Analytical methods would include various combinations of EPA method nos.
- 8015 modified, 8021b, and 8260b (limited to BTEX and fuel oxygenates). The drilling
will be completed with either a push probe or hollow stem auger rig.
Attached are the following:
• A site sketch showing approximate exploratory drilling locations
• A completed boring permit application
• A completed Licensed Contractors Declaration form
• A completed MFR form (by property owner)
• Letter from property owner authorizing the drilling/permitting
• Two checks for permit and inspection fees ($89 and $279)
We were hoping to complete this work next Thurs/Friday (6/10 & 6/11) based on rig availability.
Can the permits be processed by this time, or can the work proceed with a verbal approval of
permits? We are located in Southern California and need to make travel arrangements. Please
call me if you need any additional information (909) 476-2120.
Very y Yo s,
A A EM
BERT B. HANSEN
Project Env.
