SANJ O A Q t ,I N Environmental Health Department <br /> C0UN-TY <br /> Complaint Record Daily Time and Activity Summary from 6/21/2013 to <br /> 6/21/2020 for C00041250 located at <br /> 2905 W BENJAMIN HOLT DR <br /> Activity Date Recorded by <br /> 03/25/2016 Received a complaint from Haza regarding a 0.5 gallon gasoline spill. Called PE 2546 MANZO <br /> 30 min Gerardo Acuna, who indicated that a spill occurred around 11:45 am and was SC 544 <br /> cleaned up by the site right away, a top off from the customer. Called the store, no <br /> manager is on duty and no worker who witnessed the spill was present. Received <br /> a call back from Chantal Brown, who indicated that they had a spill between 11 <br /> and 11:30 it was a large spill and went onto two dispensers. They cleaned the spill <br /> with absorbent<1/2 drum was generated, no drains were impacted. Discussed the <br /> spill with Michelle H., (Lead Sr.), we should witness the spill clean up. Left to the <br /> site. <br /> 03/25/2016 Discussed the spill with Michelle H, I will need to go to the site and verify clean up. PE 2546 MANZO <br /> 78 min Spoke with Ashreen regarding generating a complaint number for this spill. Left to SC 944 <br /> go to the site <br /> 5:00 pm, onsite. Met with Ve Tin, who was not sure where the spill was, she called <br /> someone and obtained the information. The spill occurred at pump 3/4. Observed <br /> the area, some staining was noted, no pooled liquid was noted. Ve Tin showed me <br /> the 30 gallon container where the absorbent was placed, the container had a <br /> 11-2015 accumulation start date and had a hazardous waste label. Documented <br /> the spill. Prepared Prop 65 and the ER report. 5:45 pm, left the site. <br /> 06/22/2016 Complaint overview, downloaded inspection pictures, created picture file, printed PE 2546 MANZO <br /> 48 min ER record, created site map. Called Gerardo Acuna 925-766-6939, spoke with SC 544 <br /> Gerardo, requested disposal manifest. He will email it to me, provided my email <br /> address. <br /> 08/01/2017 Reviewed the complaint, this is a gas station facility and hazardous waste PE 2546 MANZO <br /> 30 min PR0517536, printed hazardous waste manifest, abated the complaint. Made a SC 007 <br /> copy for the facility file. <br /> Page 1 of 1 <br />
