Mr. Jack Badey - 3- 14 January 2011
http://www.waterboards.ca.gov/centraIvaIley/board decisions/adopted orders/#General.
The monitoring program should be specific for this pilot scale study.
• Contingency Plan
Please provide a contingency plan that identifies actions that will be taken if amendments
or breakdown compounds are detected in the compliance zone.
• Draft Fact Sheet
Please provide a draft fact sheet and a distribution list of interested parties. The fact sheet
will notify surrounding businesses, and property owners, and other interested parties about
the planned remediation.
• Initial Study Checklist
Please complete an Initial Study Checklist for the project. Central Valley Water Board staff
will review the Initial Study Checklist to determine if all potentially significant environmental
impacts were addressed in the CEQA document prepared for the General Order. An Initial
Study Checklist has been transmitted to AMEC Geomatrix via email.
When the above items are received in an Addendum to the Pilot Scale Study, and Central
Valley Water Board staff and UniFirst are in agreement with the contents of the Fact Sheet,
the Contingency Plan and the Monitoring & Reporting Program, then UniFirst will distribute the
Fact Sheet, and Central Valley Water Board staff will post a draft Notice of Applicability and
draft Monitoring & Reporting Program on its website. When the 30-day comment period is
closed, Central Valley Water Board staff will consider comments and either issue the Notice of
Applicability, revise it, or proceed with preparing individual Waste Discharge Requirements.
When the Notice of Applicability is issued, UniFirst may proceed as permitted.
Thank you for pursuing remediation at this site, and for posting onto Geotracker past reports,
monitoring well logs and locational information. If you have any questions, you may reach me
by telepho at (916)464-4680 or by email at aterrell(awaterboards.ca.gov.
A Y T RRELL
Private Sites Cleanup Unit
Enclosure
cc: Mr. Michael Infurna, San Joaquin County Environmental Health Department, Stockton
Mr. William Pipes, AMEC Geomatrix Consultants, Fresno
