Please be advised that the following issue(s) has been addressed: <br /> Secondary containment testing was last performed on 3/24/08 and was due by 3/2011.Testing has not <br /> been completed and is one month past due.Secondary containment testing is required once every 36 <br /> months. Immediately schedule this test and provide 48 hours notification to the Environmental Health <br /> Department(EHD). On April 29, 2011,our testing vendor,Tank-Tek, performed a passing test. A copy of the <br /> test results are attached for your review and records. <br /> The 91 octane fill spill container failed when tested.All spill containers shall have a minimum capacity <br /> of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately contact a properly licensed,trained,and certified contractor to repair or replace the spill <br /> container under permit and inspection to the EHD. On April 19, 2011, our repair vendor, Fastech, repaired <br /> the 91 octane fill/spill container. A copy of the passing test results is attached for your review and records. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste.The facility has <br /> the following waste stored on site: a.Two 1-gallon containers of oily liquid stored on top of a <br /> hazardous waste drum. Immediately make a hazardous waste determination for the above waste,and <br /> manage it according the Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how these wastes were managed by May 5,2011. The contents of the two <br /> 1-gallon containers were placed into the liquid hazardous waste drum. The hazardous waste drum was <br /> removed by an authorized hauler on April 18, 2011. A copy of the manifest is attached for your review and <br /> records. <br /> One partially full 55-gallon drum is labeled "hazardous waste,solids containing flammable liquid,toxic <br /> absorbents,accumulation date 1-20-08".The drum is missing the generator name and address.All <br /> hazardous waste containers shall be marked with the following information: 1.the words"hazardous <br /> waste". 2. name and address of generator.3. hazardous properties.4. physical state.5.composition <br /> (contents). 6.accumulation start date. Immediately label these containers and ensure that all hazardous <br /> waste containers are marked with all the required information. We confirmed that the Designated <br /> Operator affixed a properly completed label to the drum prior to its removal on April 18, 2011. <br /> Page 1 of 2 <br />
