This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br />generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br />an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br />evidence to the EHD by 12-4-11. <br />128. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br />copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br />bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br />EHD by 12-4-11. <br />301. Oily debris is on the floor of the petroleum products storage shed. Facilities shall be maintained and <br />operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or <br />surface water which could threaten human health or the environment. Immediately clean up the oily <br />debris, and manage according to Title 22 hazardous waste regulations. Submit a statement and <br />supporting documentation explaining how this waste was managed by 12-4-11. <br />404. Three two gallon pails of used oil were left open in the vehicle shed. All hazardous waste containers <br />shall be closed at all times except when adding or removing waste.
