Park Ave. and 2801 Louise Ave. <br /> Lathrop CA 95330 Lathrop CA 95330 Lathrop CA 95330 <br /> APN #198-10-18 APN #198-09-17 APN #198-110-02 <br /> On August 13, 1998 and August 14, 1998, thirteen direct push bore holes were advanced at the above <br /> referenced properties as part of an environmental assessment which was performed by Advanced <br /> GeoEnviron mental, Inc. (AGE) for California Natural Products, Inc. A work plan dated July 23, 1998 <br /> detailing the scope of work, and an addendum dated August 10, 1998 which added the third parcel, had <br /> been received by San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD). <br /> Permits#016516 and #016517 were issued by PHS/EHD on August 7, 1998, and permit#016559 was <br /> issued on August 11,1998. PHS/EHD personnel were present to observe the grouting of the bore holes, <br /> as per section 13.17.11 of the San Joaquin County Well Standards. <br /> On December 9, 1998 PHS/EHD received a copy of the report"Results of Grab Ground Water Sampling <br /> and Analysis, 2020 E. Park and 2440 E. Louise Ave., Lathrop, California" which was prepared by AGE for <br /> Mr. Pat Mitchell of California Natural Products, Inc. Upon review of the report it was noted, in section 4.0 <br /> Procedures, that "Additional sample was collected from one sampling point on 25 August 1998". After <br /> several phone conversations between AGE and PHS/EHD it was verified that on August 25, 1998, Staff <br /> Geologist William Little of AGE hand augured an additional boring on site to obtain a water sample for <br /> EPA method 8270 analysis.
