CONTINUATION -OFFICIAL INSPECTION REPORT <br />QBR, 2325 W. Charter Way, Stockton, Ca 95206 <br />The Unified Program Hazardous Waste Inspection report dated 5/10/00 has been amended to include <br />violation number 4, 12, 24, 28, 75 b and 75 c. <br />NOTICE OF VIOLATIONS <br />4. Disposed of hazardous waste at an unauthorized point. <br />Floor sweep containing sand -blast beads was disposed in a trash can in Caliper Area. <br />Effective immediately, all hazardous waste shall be manifested and disposed of in accordance with <br />Title 22 and Health & Safety Code. <br />HSC 25189.5 <br />12. Facility not maintained to minimize the release of a hazardous waste. <br />In Caliper Area at Rust -Arrest Station, oil was dripping from calipers into shallow drip pans and on to <br />the floor. <br />Effective immediately, maintain the facility to minimize the release of hazardous waste and implement <br />spill prevention control and countermeasure plan. <br />CCR 66265.33 <br />16. Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br />The employee who handled hazardous waste did not have communication equipment as described by <br />the plant manager. There was no decontamination equipment at all hazardous waste storage area. <br />a. Effective immediately, provide communication equipment at all hazardous waste storage area or <br />ensure that the employee who handles hazardous waste has communication equipment. <br />b. Within 1 week from the date of receipt of this report, provide decontamination equipment at all <br />hazardous waste storage area and hazardous waste generation site. <br />CCR 66265.33 <br />20. Failed to properly label containers of hazardous waste. <br />Hazardous waste container at satellite accumulation points in Caliper Area did not hazardous waste <br />label. <br />Effective immediately, provide hazardous waste label that contains all information listed in CCR <br />66262.34 on all hazardous waste containers. <br />CCR 66262.34 <br />24. Manifest not properly completed, signed or dated. <br />The generator did not sign several manifests. A person other than generator signed those manifests. <br />Effective immediately, all manifests shall be signed by the generator in accordance with the <br />instructions in the Appendix to chapter 12 of CCR. <br />CCR 66262.23 (a) (1) <br />28. Failed to determine status of hazardous waste when manifest copy not received. <br />Manifest number 97332895 dated 12-28-98, 97332897 dated 05-06-99 and 97332898 dated 7-12-99 <br />were signed by Joseph Moreno, QBR HR Manager, in box 20 of the manifest as the receiving party at <br />TSDF. <br />Within 7 days of receipt of this report, submit proof that the waste was taken to a permitted treatment, <br />storage, and disposal facility. <br />CCR 66262.42 (a) <br />45. Contingency plan was incomplete. It did not contain all information listed in CCR 66265.52 <br />Within 30 days of receipt of this report, update the contingency plan to include all information listed in <br />CCR 66265.52 and 66265.56 <br />5 of 6 <br />j5j4> <br />
