ACTIVITY HAZARDS ANALYSIS <br /> JOB STEPS HAZARDS ACTIONS TO ELIMINATE OR MINIMIZE HAZARDS RAC <br /> •Prior to using contractors,follow the rules and requirements <br /> contained in,and which provides guidance for,Contractors performing <br /> work under contract. <br /> •Use spotters when positioning vehicle if needed. Ensure spotters <br /> know how to communicate with driver of vehicle. <br /> •Locate vehicle in an area that will not obstruct traffic. Use flaggers and <br /> Location could create a traffic hazard. post signs as required by the DOT Uniform Manual of Traffic Control <br /> 3.Park underground locator contractor vehicle at site. Devices(UMTCD)and Cal-Trans requirements. <br /> 4.Unload equipment from vehicle. Vehicle could hit someone or something. .Use proper lifting techniques such as keeping the back straight,lifting <br /> Heavy manual lifting. p p g q p g g g <br /> 5.Move equipment to designated survey location. with legs,limiting twisting,and getting help when moving bulky/heavy M <br /> Lifting of test instruments from vehicle could f materials and equipment. Use hand truck if needed. For loads greater <br /> cause strain worker. than 50 pounds,use two people to lift. <br /> Slip,trip,and fall hazards could be present. .Visually inspect work areas and mark,barricade,or eliminate slip,trip, <br /> and fall hazards. Only work on walking/working surfaces that have the <br /> strength and integrity to support employees safely. Openings 18 <br /> inches or more in diameter must be covered and marked. All openings <br /> less than 18 inches in diameter and all holes must be marked or <br /> barricaded. <br /> Wear high-visibility reflective vests at all times in work areas. Make <br /> eye contact with operators of vehicles.Post an observer,as needed, <br /> when surveyor is using instruments(a surveyor is often focused on the <br /> task and may not be aware of nearby traffic). <br /> -Use traffic controls or barricades,if necessary,to keep traffic away from <br /> workers.Follow Cal-Trans requirement <br /> Follow manufacturers'instructions on use of paint.Review MSDSs. <br /> 6.Survey and mark utilities Worker could be struck by vehicles. Never point spray paint canisters at another person. M <br /> Use following universal color codes for utilities:Blue-Water;Red- <br /> Electrical;Yellow-Gas;Green-Sewer. <br /> Carry stakes in leather or canvas bag that is puncture proof,and carry <br /> bag to side of body.Ensure all tips are pointed toward ground at all <br /> times. <br /> Keep stake tip pointed at ground.Wear leather gloves. Use caution <br /> when using tools to pound stake in.Ensure workers are wearing safety <br /> glasses. <br /> Obey traffic signs and signals. <br /> 7.Driving on and off site.
