APP/SSHP <br /> Soil Excavation at California National Guard Armory <br /> Lodi, California <br /> 1.0 INTRODUCTION <br /> 1.1 PURPOSE <br /> This Site Safety and Health Plan (SSHP) describes the procedures to be followed in order to <br /> reduce employee exposure to potential health and safety hazards that may be present at the <br /> project site. The emergency response procedures necessary to respond to such hazards are <br /> also described within this SSHP. <br /> The purpose of this SSHP, working in conjunction with the project APP, is to provide Oneida <br /> Total Integrated Enterprises, LLC (OTIE) field personnel, subcontractors, and other visitors with <br /> an understanding of the potential chemical, biological, and physical hazards that exist or may <br /> arise while the tasks of this project are being performed. The primary objective is to ensure the <br /> well being of all field personnel, the community, and the environment surrounding the site. <br /> OTIE personnel have the authority to stop work performed by our personnel or our <br /> subcontractors at this site if work is not performed in accordance with the requirements <br /> of the site APP/SSHP/AHAs. <br /> All OTIE employees and subcontractors who perform fieldwork during the project will be <br /> required to read this APP/SSHP and acknowledge receipt and understanding of this SSHP by <br /> signing the APP/SSHP Compliance Agreement (Appendix E) and submitting it to the OTIE <br /> Project Manager(PM) before performing any field activities. <br /> The SSHP will be periodically reviewed and modified throughout the duration of the project to <br /> ensure flexibility and adaptability as changes occur and new situations develop. Modifications to <br /> the SSHP must be approved by the OTIE PM, the Field Team Leader (FTL), the Site Safety and <br /> Health Officer(SSHO) and the Safety and Health Manager(SHM). Any changes will be <br /> attached at the end of the APP/SSHP, in Appendix J. <br /> 1.2 SCOPE OF THE SSHP <br /> The SSHP addresses all phases of field operations at the site, including: <br /> • Site characterization, including the history of the site; <br /> • Responsibilities of key site safety and health personnel; <br /> • Work practices and standard operating procedures (SOPs); <br /> • Hazard identification and assessment, including chemical, physical, biological, and <br /> radiation hazards; <br /> • Identification of personal protective equipment (PPE); <br /> • Exposure monitoring/air sampling procedures; <br /> • Heat stress monitoring; <br /> • Response procedures for accidents and emergencies; <br /> • Emergency contacts and phone numbers; <br /> • Medical surveillance; <br /> • Training requirements for workers and on-site training; and <br /> • Record-keeping requirements. <br /> OTIE 1 <br />
