APP/SSHP <br /> Soil Excavation at California National Guard Armory <br /> Lodi, California <br /> 4.4 SUMMARY OF ACTIVITY HAZARD ANALYSIS (GENERAL) <br /> Activity Hazard Analysis (AHAs) for the site are contained in Appendix B to this APP/SSHP. <br /> These include the potential hazards associated with each specific activity and the <br /> recommended controls. AHAs expected for this project are included in Table 1 in the APP and <br /> repeated here. <br /> Table 1. List of AHAs <br /> AHA Number Description <br /> 01 Mobilization <br /> 02 Underground Utility Clearance <br /> 03 Soil Sampling <br /> 04 Soil Excavation <br /> 05 Off-Site Waste Transport and Disposal <br /> 06 Demobilization <br /> The levels of precautions or controls will be upgraded or downgraded as appropriate, based on <br /> air monitoring data and site conditions. These changes will be initiated by the SSHO and <br /> approved by the PM and the SHM. No confined space work exists for this project and none will <br /> be created. All of these AHAs are subject to review and revision once subcontractors are hired <br /> to perform the various tasks as they will update the safety procedures specific to the tasks and <br /> equipment at the site. These changes will be initiated by the SSHO, reviewed by the cognizant <br /> POC office, and approved by the PM and the SHM. <br /> 4.5 PHYSICAL HAZARDS <br /> Potential physical hazards that may be encountered during work at the site include the <br /> following: <br /> 4.5.1 Electrical and Utility Hazards <br /> OTIE personnel and subcontractors will operate and maintain power supply units. All field <br /> personnel working in and around electrical hazards will be experienced engineering technicians <br /> that are familiar with the hazards of energized sources. <br /> Hand-held power tools will be used as needed, and each tool will contain properly grounded <br /> three-wire electrical cords. <br /> Prior to all scheduled work activities all electrical power must be isolated in order to prevent the <br /> transmission or release of energy. This isolation for the control of hazardous energy is <br /> commonly called "Lockout/Tagout". All provisions of the OSHA standard as stated in 29 CFR <br /> 1910.147 and EM 385-1-1 Section 12.0 must be followed for controlling hazardous energy or <br /> locking out or tagging out electrical and utility hazards. OTIE employees must follow Section <br /> 25.0 "Control of Hazardous Energy (Lockout/Tagout)" of the OTIE Corporate Safety and Health <br /> Manual. <br /> Electrical hazards exist from the use of electrical equipment at the site. All electrical equipment will <br /> be visually inspected for external defects to the equipment, including the cord. Should there be any <br /> evidence of damage, that equipment is not to be used. <br /> OTIE 10 <br />
