The State Water Resource Control Board(SWRCB)will invoice you directly for activities performed <br /> under the auspices of the Local UST Oversight Program. The goal of the site evaluation performed by Public Health <br /> Services Environmental Health Division (PHS/EHD) is to certify closures of underground tank leakage sites. <br /> Closure certification occurs only after mitigation activities are completed. <br /> i <br /> In addition, the"Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground <br /> Tanks" and "Appendix A" (copies enclosed) reportingformat and the LUFT Manual shall be utilized as guidance _ <br /> documents in the preparation of work plans and/or reports. <br /> I <br /> A copy of the Corrective Action Requirements from the California Underground Storage Tank Regulations, <br /> California Code of Regulations, Title 23, Division 3, Chapter 16 is enclosed as well as an application for the <br /> California Petroleum Underground Storage Tank Cleanup Fund. <br /> If you have any questions or there are any corrections in the responsible party information, contact Margaret Lagorio <br /> at (209) 468-3449. <br /> Donna Heran, REHS, Director <br /> Environmental Health Division <br /> Margaret Lagorio, Lead Senior REHS Diane .
