Mel Bokides Petroleum, Inc <br />' 501 W Lodz Avenue, Lodi, California <br />' 1.0 INTRODUCTION <br /> At the request of Mr Nicholas Bokides of Mel Bokides Petroleum, Inc , Advanced <br />' Geo-Environmental, Inc (AGE) has prepared this Corrective Action Pian (CAP) for 501 W Lodi <br /> Avenue (site), Lodi, California The site setting is illustrated on Figure 1 Site structures and <br /> monitoring well locations are depicted on Figure 2 <br /> This CAP has been prepared as required by the San Joaquin County Public Health Services - <br />' Environmental Health Division (PHS-EHD) for the mitigation of petroleum hydrocarbon impacts <br /> to soil and ground water related to former leaking underground storage tanks (USTs) at the site The <br /> CAP was prepared in accordance with guidelines established by the Tri-Regional Board Staff <br /> Recommendations for Preliminary Investigations and Evaluation of Underground Stol age Tank <br /> Sites(RWQCB, 1990)and the California Water Resources Control Board's California Undeigi ound <br /> Storage Tank Regulations (CWRCB, 1992) <br />' The purpose of the CAP is to present andsynopsis the petroleum hydrocarbon impacts in soil and <br /> ground water at the site and evaluate feasibility and cost aspects of various remedial action <br /> alternatives <br />' 20, BACKGROUND <br />' The site was formerly operated as an Arco service station which stored and distributed gasoline fuel <br /> for retail sale <br /> 1 2 1 SITE DESCRIPTION <br />' The site is located at the northwest corner of the Lodi Avenue and Hutchins Street in Lodi, <br /> California The site was operated as a gasoline dispensing station until February 1998,when the US f <br /> system was removed The facility operated as a service station for intermittent durations from the <br />' 1950s until Februaiy 1998 MBP purchased the site in 1985 to operate the site as a fuel service <br /> station From 1949 to 1966 the site was operated as the Tide Water Associated Oil Company and <br /> in 1966 the site came under operation by the Philips Petroleum Company Between 1966 and 1985 <br /> the occupants have not been determined After 1985'the site was operated as a fuel service station <br /> Advanced GeoEnN tronmental,Inc <br /> 1 <br />
