INTRODUCTION <br /> At the request of Mr Nicholas Bokides of Mel Bokides Petroleum, Inc , Advanced <br /> GeoEnvironmental, Inc (AGE) has prepared this Quarterly Report for 501 W Lodi Avenue (site), <br /> Lodi, California The site setting is illustrated on Figure 1 Site structures and monitoring well <br /> ' locations are depicted on Figure 2 <br /> ' The scope of work was to install four soil borings,three ground water monitoring wells,and two soil <br /> gas/vapor wells on the site The objective of the work was to monitor petroleum hydrocarbon <br /> concentrations in ground water and to characterize hydrocarbon impacted to ground water on the <br /> ' srte <br /> Field work was performed and this report was prepared in accordance with guidelines established <br /> ' by the Central Valley Regional Water Quality Control Board (CVRWQCB) and by the San Joaquin <br /> County Public Health Services - Environmental Health Division (PHS-EHD) for the subsurface <br /> investigation of former underground storage tank(UST) sites <br /> A summary of previous investigations at the site was included in the Work Plan far Site Assessment, <br /> Mel Bokides Petroleum, 501 West Lodi Avenue, Lodi, Califoi nia, dated 15 May 1998, prepared by <br /> ' AGE <br /> ' 20. PROCEDURES <br /> ' Initially,the scope of work for the ground water investigation at the site included the installation of <br /> two ground water monitoring v�ells, two soil gas/vapor extraction wells and four to five additional <br /> soil borings Hove ever, due to higher than expected ground water contamination in the former UST <br /> ' area, an additional ground monitoring water well was installed in the central area of the former UST <br /> excavation at the direction of the PHS-EHD personnel on site during field inspections The well <br /> allows for the repeatable sampling of the ground water in the former UST area to better assess the <br /> migration of the petroleum h)drocarbons at the site <br /> 21 DRILLING AND SAMPLING <br /> On 28, 29 and 30 October 1998, three ground water monitoring wells, two soil gas/vapor wells and <br /> ' four soil borings were installed on-site Drilling and sampling was performed using a truck-mounted <br /> AdtancedGeoEnNironmental,Inc <br />
