The following is an itemized list of hazardous materials violations that have not been <br /> addressed for MIKASA ASIAN BISTRO & SUSHI BAR as of September 24, 2020. <br /> Open violations from February 26, 2020 inspection <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-Failed to annually review and certify that the business plan is complete,accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.The business plan was last submitted on 6/8/2017.The business plan has not been <br /> submitted for the years of 2018, 2019, and 2020. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/,enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were unavailable for review at time of inspection. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release <br /> of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: <br /> (A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C)Evacuation plans and procedures, including immediate notice, for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> Page 3 of 4 <br />
