Permission to conduct inspection was granted by Curtis Williford (Automotive Technician II). <br /> 2.)TPID certificate is posted in the shop office. <br /> 3.)Waste tires were counted directly. <br /> 4.) No activity on WTMS. Manifests for pickups on 10/11/2016, 06/21/2016, 01/28/2016, 11/12/2015, 04/06/2016 <br /> and 09/17/2015 from "Gutierrez Tire&Wheel Inc."were on site and available for review. <br /> 5.) Retain all waste tire logs, receipts, and manifests for 3 years. <br /> 6.) Facility only stores its waste tires at this location. <br /> 7.) Inspection report emailed to cwilliford@chp.ca.gov. <br />
