401 Lincoln <br /> Page 2 of 2 <br /> Conduct a complete well survey of domestic, muni ipal, agricultural, industrial and any <br /> other wells within a 2,000 foot radius of this site, Contact the Department of Water <br /> Resources, municipal water suppliers, and any nea by industries for well information. <br /> PHSIEHD well permit records should be researched and a physical search of the area to <br /> identify any wells should be performed. The results of the well survey should be <br /> submitted in a report to PHSIEHD. Identify all sources of information in the well survey <br /> report. Include a map that clearly depicts each well location with respect to the above <br /> referenced site and provide any known constructio 1 details of the wells. <br /> Perform a conduit study that details the potential ru igration pathways and potential <br /> conduits (sewers, storm drains, etc.) that may be present in the vicinity of this site. <br /> Provide a map showing the location and depth of a i utility lines and trenches including <br /> sewers and storm drains within or near the contaminated area. Please include any prints <br /> of historic aerial photos used as part of your study, and use data tables and maps to report <br /> the data collected as part of your survey.
