and dated October 20, 1995 . <br /> 2. 1. 1 Health and Safety Plan <br /> Prior to initiating any field work, a site specific health and safety plan was prepared in <br /> accordance with 29 CFR 1910. 120 . The health and safety plan provides a summary of physical <br /> - and chemical hazards at the site, names and telephone numbers of responsible individuals <br /> working on the project, and emergency instructions and directions to receive medical attention. <br /> An abbreviated version of the health and safety plan without its attachments is included as <br /> Appendix A. <br /> 2. 1. 2 Permits and Utility Clearance <br /> To perform the soil and groundwater investigation, a Well/Pump Permit was obtained from <br /> PHS/BHD . A copy of the permit is included in Appendix B . <br /> Since all drilling locations were located within the airport property boundary and off of any <br /> public road right-of-way, neither offsite access agreements nor encroachment permits were <br /> necessary, <br /> At least 48 hours prior to drilling operations, Underground Service Alert (USA) was notified to <br /> perform a routine utility clearance inspection. As an added precaution, each sampling location <br /> was dug by hand to a depth of approximately 4 feet bgs to prevent any damage to underground <br /> utilities. <br /> 2. 2 Soil and Groundwater Investigation <br /> On January 3 and 4, 1996, IT Corporation conducted an investigation of subsurface soil and <br /> groundwater using a Geoprobe Direct Penetration Technology (DPT) rig. Soil and groundwater <br /> samples were collected from two locations indicated as SB-3 and SB-4 on the Site Plan (Figure <br /> 2) . Two additional sample locations (SB- I and SB-2) were discontinued at a depth of 16 feet bgs <br /> i <br /> due to rig refusal resulting from contact with a subsurface concrete stab used to anchor the <br /> i former USTs.
