Following development of the new well,all of the monitoring wells on site will be sampled,and routine quarterly <br /> monitoring will be initiated. <br /> COMMENTS AND CONDITIONS: <br /> 1. Work Plan proposes to install a conductor casing to approximately 20-feet below surface grade,cross contamination <br /> issue. This may be necessary,but the boring Iog for MW 1 shows only a one foot clay tens at 19-feet bsg. <br /> 2. Vertical definition is generally considered to have been made when two successive soil samples are ND. <br /> 3. RP has been directed to conduct a well survey for this`site. Results of the well survey are to be submitted with the <br /> report of findings from this proposed phase of work€The report of findings is to be submitted to PHS/EHD within <br /> 60-days following completion of the groundwater monitoring. <br /> 4. Submit to PHS/El-D a well permit application and required$89.00 permit fee. <br /> 5.
