Ms. Duncan approved the work plan on June 28, 2000. <br /> ' A delay in the work plan implementation occurred due to a change to the Responsible Party. <br /> ' Cost pre-approval was obtained on October 28, 2001. <br /> In April of 2002, GTI and V&W Drilling installed one deep monitoring well to 35 feet <br /> below grade (MW-101). On June 18, 2002, GTI prepared and submitted the Site <br /> Characterization, Monitoring Well Installation, and Groundwater Monitoring Report <br /> documenting the work performed during April of 2002. The report recommended <br /> ' continued groundwater monitoring and testing for biodegradation indicators. The <br /> recommendations were approved by Ms Lori Duncan, SJC PHS/EHD, in a letter dated July <br /> 22,2002. <br /> ' On September 30, 2002, GTI prepared and submitted the 3`d Quarter 2002 Groundwater <br /> Monitoring Report recommending the site be considered for closure. The request was <br /> denied by Ms Duncan in a letter dated March 3, 2003. A meeting was scheduled between <br /> Mr. and Mrs. Sinclair, Ms Duncan and Eric Price of GTI for March 27, 2003 to discuss the <br /> direction of the project and remediation alternatives. <br /> On March 3 2003 Lori Duncan SJC PH <br /> S/EHD, directed Sinclair Trucking to develop a <br /> ' site conceptual model. The following report incorporates data obtained during the project <br /> thus far, such as sensitive receptor information, interpretation of geologic formations at the <br /> site and how they contribute to transportation of contaminants, analytes al data, <br /> quantification of contamination known to be present, and df <br /> d transport model) <br /> " wl�.�•-t ,�,, tib- r <br /> ' 2.0 SOURCES OF CONTAMINATION <br /> 2.1 Historical Sources of Petroleum Hydrocarbons <br /> ' The Sinclair Trucking site formerly stored leaded gasoline on site in a 350-gallon UST. The <br /> UST was removed on March 31, 1987 and one soil sample was collected on April 1, 1987 <br /> from the excavation. ...TPH-G, .TEPH-D, .EDB and lead were detected in the original = -= <br /> sampling event. The location of the former tank is shown in Figure 2. Contamination was <br /> detected during the tank pull and was the source of historical petroleum hydrocarbons found <br /> ' at the site. <br />
