Personal Protective Equipment Failure: If any site worker experiences a failure or <br /> alteration of protective equipment that affects the protection factor, that person and <br /> his/her buddy shall immediately leave the Exclusion Zone. Re-entry shall not be <br /> permitted until the equipment has been repaired or replaced. <br /> L Other Equipment Failure: If any other equipment on-site fails to operate properly, <br /> the Project Team Leader and Site Safety Officer shall be notified and then determine <br /> the effect of this failure on continuing operations on-site. If the failure affects the <br /> L safety of personnel or prevents safe completion of site activity, all personnel shall <br /> leave the work zone until the situation is evaluated and appropriate actions taken. <br /> Accident Report Form: Write down all circumstances surrounding the incident which <br /> caused the injury including, but not limited to: time of day, working conditions <br /> (weather, etc.), how long it had been since the last rest period occurred, what the <br /> person was doing when injured, what all other personnel on site were doing, what <br /> level of protection was being used, if all safety procedures were being followed, etc. <br /> All team members that witnessed the incident should write down their recollections of <br /> L the incident and give them to the Designated Site Safety Officer, who shall then fill <br /> out a Terranext Accident Report form (Attached). This report should be submitted to <br /> r the Corporate Health & Safety Officer and the Project Manager. <br /> Lr <br /> In all situations when an on-site emergency results in evacuation of the Work <br /> Zone, personnel shall not re-enter until: <br /> 1. The conditions resulting in the emergency have been corrected. <br /> 2. The hazards have been reassessed. <br /> C 3. The Site Health and Safety Plan has been reviewed. <br /> L� 4. Site personnel have been briefed on any changes in the Site <br /> CHealth and Safety Plan. <br /> 2.
