CONTINUATION FORM <br />Page: 2 of Z <br />.•FICIAL INSPECTION REPORT <br />Date: 05-30-06 <br />Facility Address: 1521 W. CHARTER WAY <br />Program: HAZ <br />HAZARDOUS WASTE INSPECTION <br />"NOTICE TO COMPLY" <br />#12) The waste oil collection bin observed in the hazardous waste area was overflowing with items (containers, <br />filters) being drained of fluid. Remove these overflowing items to minimize the release of hazardous waste. <br />Correct immediately. <br />#16) Waste oil was observed in the secondary containment of the 55- gallon waste oil drum located in the field <br />service shop. Secondary containment should be kept clean and dry at all times. Remove this spillage immediately <br />and handle in accordance with Title 22 Hazardous Waste Regulations. <br />#20) The following containers were observed without proper hazardous waste labeling: <br />- 5 gallon waste oil bucket located next to parts washer in fabricating shop had no label <br />- 55 gallon drum of waste oil sludge in the MIRT washer area had no label <br />- Used antifreeze in 35 gallon container in hazardous waste accumulation area was labeled as "Used <br />Oil" <br />- 65 gallon garbage can filled with spent absorbent pads and filters did not have a label <br />- Yellow waste oil draining container in the new machine shop was observed to have no accumulation <br />start date <br />- Blue used oil container in truck shop did not have a label <br />All containers of hazardous waste must be labeled with the following: <br />- The words "Hazardous Waste" <br />- Facility Name & Address <br />- EPA ID Number <br />- Accumulation Start Date <br />- Hazardous properties <br />- Contents <br />Properly label all containers of hazardous waste in accordance with Title 22 Hazardous Waste Regulations. <br />Correct immediately. Repeat Violation. <br />#21) Antifreeze container in the hazardous waste storage area as well as one in the truck shop were observed to <br />have no lid. All containers of hazardous waste must remain closed except when adding or removing hazardous <br />waste. Close these containers immediately. <br />#28) Manifest #24528779 dated 01/06/06 did not appear to have a signed copy associated with it. If the signed <br />copy of a manifest is not received within 35 days of the date that the waste was accepted by the initial <br />transporter, it is the generator's responsibility to determine the status of the waste. Immediately determine the <br />status of this waste and if necessary, file an exception report. Submit a copy of the signed copy of the manifest or <br />the exception report to this office by 06-30-06. Repeat violation. <br />#49) Personnel training records did not have job titles for each person on the training record. Also, job <br />descriptions for each position could not be located at the time of inspection. Maintain personnel training records in <br />accordance with Title 22 Hazardous Waste Regulations.
