Seismic/Anchoring: <br />Seismic/dynamic/hydrostatic load calculations provided with "Hydrostatic <br />and Seismic Analysis" page or as Attachment 3 <br />Seismic/Anchoring findings: The tank as designed and operated, and the <br />containment area in which it is located are adequately anchoed and meet <br />minimum seismic safety standards. Initials: ,r w <br />Leak Test: <br />Brief description of leak test, including type of test used, and results of <br />test: This tank was integrity tested based on recommendations from the <br />manufacturer and the Steel Tank Institute's Standard SP001, "Standard for the <br />Inspection of Aboveground Storage Tanks" (STI SP001). This standard provides <br />recommended integrity testing schedules based on risk to the environment, <br />considering factors such as spill control, release detection methods, and storage <br />capacity. Table 5.5, "Table of Inspection Schedules", of STI SP001 recommends <br />tanks of this size and type undergo periodic visual, documented inspections to <br />assess general AST conditions. These inspections may be conducted by the <br />AST owner and without suspending AST operations or removing the AST from <br />service. Holt of CA conducts these types of inspections on a monthly basis. The <br />tank inspector reviewed Holt of CA inspection logs and conducted his own <br />assessment based on the attached checklist. The tank was deemed to be in <br />proper working order and the integrity sound. A detailed description of the <br />integrity testing standard used as well as sample inspection logs are located in <br />Attachment 2. <br />Integrity testing findings: Based on the above noted integrity test and the <br />results obtained from that test, the tank has sufficient integrity for its intended <br />use.
