(Bureau Veritas) prepared this Draft Limited Subsurface Investigation <br /> Report(LSI) on behalf of McGuire Woods, LLC regarding the proposed Central Lodi cell site located at <br /> 114 North Main Street in Lodi, San Joaquin County, California(the"subject property'). <br /> This LSI documents the activities completed at the subject property, as outlined in our proposal number <br /> 3303.11.243, dated April 26, 2011,which were conducted to investigate findings identified in our Draft <br /> Visual Inspection Report(VSI), dated March 22, 2011. This investigation was performed according to the <br /> scope of work outlined in Section 2.0 of the Environmental Due Diligence Scope of Work in the <br /> Environmental Services General Agreement between McGuire Woods, LLC and Bureau Veritas dated <br /> December 29, 2006. 4,, ;.- <br /> 1.1 SITE DESCRIPTION AND PROPOSED ACTION <br /> The subject parcel is developed with four buildings: three in the northeastern portion that are used by the <br /> City of Lodi Parks and Recreation Department for offices and landscaping equipment storage, and one in <br /> the southwestern portion that is currently vacant and formerly housed the City of Lodi police and fire <br /> departments and town hall. Other features include a diesel aboveground storage tank in the northern <br /> portion used by the Parks and Recreation Department for fuel, a water tower and well in the northwestern <br /> portion, adjacent to which are two telecommunications sites operated by Verizon Wireless and AT&T; and <br /> several areas of outdoor equipment storage. The subject parcel is bordered to the west by North Main <br /> Street, to the north by Locust Street, to the east by North Stockton Street, and to the south by an alley. <br /> The subject property is located just east of the existing Verizon Wireless telecommunications site. <br /> The proposed action consists of installation of a new back-up diesel generator with an approximately 210- <br /> gallon diesel belly tank on an existing approximately 50 square foot rectangular pad east of the water tank <br /> and just north of the AT&T shelter. In addition, approximately 70 feet of underground conduit is proposed <br /> to be installed in a below grade trench, to be excavated onsite, and will extend north from the generator, <br /> and then west and south to the existing Verizon Wireless equipment shelter <br /> 1.2 OBJECTIVE <br /> The purpose of this investigation was to assess areas of concern noted in our VSI Report dated March <br /> 22, 2011, as summarizedbelow: <br /> • Evaluate the soil quality at the subject property relative to the proposed action. <br /> 2.0 SCOPE OF WORK <br /> Bureau Veritas conducted the following tasks to complete the proposed Scope of Work: <br /> "`• Conducted pre-field activities that included preparation of a Site Health and Safety Plan (SHSP) <br /> to safely perform the scope of work. <br /> t <br />
