(use <br /> PID or FID). <br /> 2, Soil samples must be taken,from the bore holes,at five foot intervals or each time that the soil <br /> lithology changes. <br /> 3, Describe a plan for handling soil generated from drilling activities. <br /> 4, Provide a scale drawing of the site showing adjacent streets, buildings, underground utility lines <br /> (approximate scale. I inch=20 feet),past location of tank and piping systems on the property <br /> and placement of proposed soil boring. <br /> 5. Analysis for 1,2-Dib romo Ethane(EDB)should be included since it was detected in the soil <br /> sample collected beneath the tank at the time of removal. Use DHS approved methods for EDB <br /> detection. Lead analysis is not required.The levels detected in the soil sample collected the day of <br /> the tank pull do not warrant further investigation. <br /> 6, If Methyl Tertiary Butyl Ether(MTBE)contamination is found using EPA method 8020, <br /> confirm the results using EPA method 8240 or 8260. <br /> Please provide PHS/EHD with a comprehensive site health and safety plan for the above referenced work. <br /> In addition please submit a completed soil boring/monitoring well application (enclosed) and the $89.00 <br /> permit fee.
