All analytical results must come from a California certified laboratory or this office <br /> will not accept them. <br /> 2. All hazardous waste must be treated, stored or disposed of at a permitted facility. <br /> 3. See pages 7 & 8 of the permit application for required analytical tests. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. Schedule removal appointment 48 hours in advance. <br /> 6. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results, sent from the lab <br /> (b) UST Tracking Record Sheet <br /> (c) Tank Closure Certification Form <br /> (d) Hazardous Waste Manifests produced from the removal. <br /> 6. This permit expires 90 days from issue.
