CAMBRIA <br /> 0` �Lo <br /> 31�� <br /> March 3,2006 <br /> Ms Victoria McCartney <br /> San Joaquin County Environmental Health Department <br /> 304 East Weber Avenue,Thud Floor <br /> Stockton,California 95202 <br /> Re Monitoring Well Destruction Report <br /> Former Shell Service Station <br /> 1071 North Main Street <br /> Manteca,California <br /> SAP Code 135570 <br /> Incident No 98995643 <br /> SJCEHD Site Code 506313 <br /> Dear Ms McCartney <br /> Cambria Environmental Technology, Inc (Cambria) prepared this report on behalf of Equilon <br /> Enterprises LLC dba Shell Oil Products US (Shell) to document the recent destruction of three s <br /> groundwater monitoring wells at the above referenced site The scope of work followed during <br /> this investigation was presented in Cambria's February 15, 2006 Well Destruction Work Plan <br /> and was approved by the San Joaquin County Environmental Health Department (SJCEHD) in <br /> their February 21, 2006 letter to Shell This well destruction work was performed in accordance <br /> with Central Valley Regional Water Quality Control Board and(SJCEHD)guidelines <br /> SITE DESCRIPTION <br /> The site is a former service station located on the southwestern comer of the intersection of <br /> North Main Street and Louise Avenue in a commercial area of Manteca, California (Figure 1) <br /> The site layout formerly consisted of a service station building, four dispenser islands and an <br /> underground storage tank complex(Figure 2) <br /> WELL DESTRUCTION ACTIVITIES <br /> Cambria obtained well destruction permit 0045889 from SJCEHD prior to well destruction <br /> activities(Appendix A) Wells MW-1 through MW-3 (Figure 2)were destroyed on February 27 <br /> Cambria Hugh February 28, 2006, by Gregg Drilling and Testing, Inc (Gregg) of Martinez, California, <br /> Environmental under the observation of Cambria field staff With approval from SJCEHD,the above mentioned <br /> Technology,inc' wells were destroyed by pressure grouting The well casings were filled with cement grout from <br /> aPerkins Street the bottom up, a pressure seal was placed on top of the well casing,and additional cement grout <br /> oma CA 95476 <br /> Tel(707)935-4850 <br /> Fax(707)935-6649 <br />
