Hunter& Associates for the above <br /> referenced site on February 17, 1999. The work as outlined in the above referenced Work Plan has not <br /> been completed. <br /> The work as outlined in the approved Work Plan must be completed by October 20, 1999. If work is not <br /> completed within that time frame the site will not be in compliance with PHS/EHD directives and <br /> reimbursement for corrective action costs from the State Water Resources Control Board Cleanup Fund <br /> may be jeopardized. <br /> A well permit application(with encroachment permit if necessary)and $89.00 permit fee should be <br /> submitted to PHS/EHD as soon as possible. Atter the permit has been issued the work can be scheduled. <br /> Quarterly groundwater monitoring of all MWs should be initiated upon completion of MW installation and <br /> continued until otherwise directed by PIIS/EHD. The water from all MWs should be analyzed for <br /> Benzene, Toluene, Ethylbenzene, Xylenes(BTEX)and Total Petroleum Hydrocarbons as diesel (TPH-d) <br /> using EPA Method 8020. Groundwater samples should also be analyzed for Ethyl Tertiary Butyl Ether <br /> (E'I'BE), Tertiary Butyl Alcohol (TBA), Methyl Tertiary Butyl Ether(MTBE), Di-isopropyl ether(DI1 1-1 <br /> and 'tertiary Amyl Methyl Ether(TAME) by EPA Method 8260(see attached letter). <br /> PHS/EHD reviewed the Groundwater.Field A4onimring Summary Repos!dated December 2, 1997 prepared <br /> by William .I. Hunter&Associates for the above referenced site. Plate III of this report shows locations of <br /> wells within 400 feet of the site. Construction details and address locations for these wells were not <br /> included in this report and 400 feet is an insufficient search distance in which to determine public and <br /> environmental health risks from the contamination at this site. <br /> Conduct a complete well survey of domestic, municipal, agricultural, industrial and any other wells within <br /> a minimum 2,000foot radius of this site. Contact the Department of Water Resources, municipal water <br /> suppliers and nearby industries for well information. Perform a physical search of the area to identify any <br /> wells. Show the locations of all wells and submit any known construction details of the wells. Identify all <br /> sources of information in the well survey report and clearly depict each well location as they relate to the <br /> above referenced site. A survey should also be conducted to locate any potential conduits. Discuss <br /> conclusions regarding public and environmental health risks as they relate to the pumping influence of <br /> these identified wells and the migration and potential impact of the contaminant plume. <br /> A diagram depicting a historically varied groundwater flow direction at this site was presented in the <br /> Second Quarter 1998 Grounduvaler Monitoring Report dated August 17, 1998 prepared by William J. <br /> Hunter& Associates for the above referenced site.
