Louise Avenue <br /> Lathrop, CA <br /> San Joaquin County Public Health Services, Environmental Health Division <br /> (PHS/EHD) has received notification from the Central Valley Regional Water <br /> Quality Control Board that they concur with our determination that no further <br /> action related to the underground storage tank release is required for the above <br /> referenced site (copy enclosed) . <br /> Please submit a plan for the destruction of the monitoring wells, a PHS/EHD <br /> permit application, and $60 permit fee_ At a minimum, monitoring wells MW1, <br /> MW2, and MW3 should be drilled out and the bore holes filled with grout. The <br /> other monitoring wells may be destroyed by filling the casing with grout and <br /> applying pressure to the grouted casing.
