a. Bidding Requirements i Prospective bidders must be agreeable to, or meet, the following specifications: 1. Consultant must have at least 5 years experience conducting under ground storage tank corrective action work in California. A statement of qualifications* attesting to such experience must accompany the bid package. r 2. Consultant must employ qualified persons and/or sub contractors to perform the work described above, including licensed professionals and other technical professionals, and possess all necessary licenses to operate within the State. 3. Consultant must carry out the described work in a timely, professional, safe and legal manner and maintain necessary records and other safeguards to ensure that reported sample results are accurate. All work must be performed and reported in a manner acceptable to the DEH. 4. Consultant must provide at least 5 references for whom the consultant has performed similar work, in the last 5 years. This must be provided with the bid package, and should include names and phone numbers of the person to verify the work reference. 5. Consultant must maintain at least 1 million dollars in General Liability Insurance, and in addition must provide and maintain at least 1 million dollars in Professional Liability Insurance ,(E & O), including accidental pollution coverage. Proof of insurance must accompany the bid, and a telephone number and a contact person at the insurance & O insurance will must be included. No exceptions tions to the E I company p be considered. 6 Work shall be performed in a manner which will not interfere with the normal operations of the Manteca Unified School Districtunless absolutely necessary. I - 7. Consultant shall provide a copy of their standard services agreement and schedule of charges with any bid submitted.. The Bid Form provided in this bid package must be filled out. Additional information can be provided by the consultant if necessary. 3
