Main Street <br /> Manteca, CA 95336 <br /> i <br /> As the responsible party for the above referenced site you were notified by San Joaquin County <br /> Public Health Services, Environmental Health Division (PHS/EHD) in July 1997 that this site had <br /> been assigned to the PHS/EHD Local Oversight Program (LOP) for assessment and clean-up of <br /> contamination from underground storage tanks. A work plan to begin the investigation of the <br /> vertical and lateral extent of the contamination evidenced at the December 18, 1990 removal of <br /> two underground storage tanks was requested to be submitted by September 4, 1997. <br /> Written requests for the work plan made by PHS/EHD in correspondence dated March 17, 1999 <br /> and June 1, 1999, to the listed address at 5727 Clouds Rest, Mariposa CA, 95338 were met with <br /> no response. PHS/EHD then sent correspondence dated July 6, 1999 making notification that a, <br /> meeting to discuss your failure to comply with the required investigation was scheduled for July <br /> 22, 1999, at 9:00 AM, at 304 E. Weber Ave., Stockton, CA. No representative from Manteca <br /> Equipment Rental appeared for the meeting. <br /> Following the failure to appear, PHS/EHD made phone contact with Manteca Equipment Rental <br /> at the site address and was informed that the Mariposa address was still current, but that the <br /> Salmons were on vacation and had been gone for several months. The status of the site was <br /> discussed between PHS/EHD and the Manteca Equipment Rental employee who answered the <br /> phone, who had a copy of the March 17, 1999 PHS/EHD correspondence, and it was noted that a <br /> bid for a work plan had been obtained from an environmental consulting firm. PHS/EHD again <br /> requested that a work plan be submitted to investigate the extent of contamination at this site. <br /> Please note that failure to submit the requested work plan within 14 days of the date of <br /> this letter will result in referral to the San Joaquin County District Attorneys Office for <br /> enforcement action.
